If you want to enquire whether you need to have any vaccinations relating to foreign travel you will first be asked to fill in a pre-travel Questionnaire and hand this into Reception. The practice nurse will assess which vaccinations (and anti-malarials) are recommended for the countries and areas within countries that you are visiting. You will need to contact the Surgery to enquire about the recommendations for your travel.
If you are required to have vaccinations it is important to make an appointment as early as possible to have these administered. These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Please note: we are currently not able to administer Yellow Fever vaccinations or give out Certificates.
To help us offer the appropriate advice, please fill out the questionnaire within the link below and bring with you before you make your appointment. Our lead nurse will then assess the form and you will be required to call the Surgery to find out what vaccinations you need, if any, and at this stage you are can then book your appointment to attend to receive your vaccinations.
DUE TO PATIENT CONFIDENTIALITY WE CAN ONLY GIVE INFORMATION OUT ABOUT YOU AND NOT OTHER FAMILY MEMBERS, THIS INCLUDES WHETHER AN APPOINTMENT IS NECESSARY; SO EACH INDIVIDUAL WILL BE REQUIRED TO CALL FOR INFORMATION RELEVANT TO THEMSELVES.
